Transferring Care?

We want to make sure that we have all of your child’s pertinent medical information before establishing care here.

Please request records before scheduling the first appointment.

Options for requesting records:

1. Records Release (PDF) — fill this out, sign it, and either send it to us (by email, text, secure message, Athena Portal or fax) for us to send to the previous provider, or send this form to your provider yourself.

2. Use your previous provider’s forms to have records sent to us.

3. Use your previous provider’s portal to generate records and send it to us by email, secure message, Portal or fax

Once we’ve received their records, we will reach out to help you schedule your child’s first appointment.
