As our practice continues to grow and requests to complete patient forms have increased, we are updating our forms process to help us complete requests as efficiently and accurately as possible for all families.
Beginning immediately, forms that are not brought to a scheduled appointment will have a completion fee, as insurance does not cover provider/staff time spent completing forms outside of visits.
Processing options:
• Standard Processing: 4 business days — $25 per form
• Expedited Processing: 2 business days — $35 per form
• Immunization forms for school/daycare: 4 business days — $10 per child
We will make every attempt to get these done faster than the stated time. Business days do not include weekends or holidays. Fees will be invoiced to your child’s account. We will also be using a new online form request process (https://forms.gle/8vTp4oHmdjLTLSZj9).
To help avoid delays, families will be asked to:
• Fill out all parent/patient portions of the form, medications and dosing, when they take the medication, etc
• Complete the electronic request form
• Upload the pre-filled PDF form (or bring pre-filled paper form to your visit)
Incomplete forms may need to be returned for completion before processing.
To complete forms, children must:
• Have had a well visit within the past year (for our primary care patients)
• Be up to date on recommended medication follow-ups when applicable
Examples of forms include:
• School/camp/daycare medication forms
• Immunization forms for school/daycare - families can generate their own immunization certificates
• Sports and camp forms
• Allergy forms
• IEP/504 letters
• Other miscellaneous paperwork tasks
If your child has completed a well visit within the past year, sports/camp forms can often be completed using information from that visit. Insurance companies generally don’t provide coverage for an additional sports physical.
We will send back the completed form to the family by text or secure message (Spruce), and the family will submit the form to the school. We will not send forms directly to schools.
